    Position description

    Under the supervision of the Manager, the Certified Tumor Registrar (CTR) is responsible for processing, maintaining, compiling, and reporting health information data to be used for research, quality assurance, facility planning, and marketing. CTRs abstract and code clinical data from medical records using appropriate classification systems, and analyze health records according to published governmental and institutional standards. CTRs read and interpret the various components of the medical record, such as pathology reports, radiology x-rays and scans, physical findings, and all treatment modalities, and use the information to correctly abstract and stage the cancer case employing various required staging systems.

    Additionally, incumbents determine the site of origin (topography) and histologic type (morphology) of the cancer and code according to the International Classification of Diseases for Oncology. This classification is distinguished from Medical records Coder series by possession of the Certified Tumor Registrar (CTR) credential, and focus/use of the material examined in the medical record.

    ESSENTIAL JOB FUNCTIONS:

     

    1. Identifies all reportable neoplasms. Abstracts required cancer related information from patient medical records by reading and interpreting pathology reports, radiology x-rays and scans, treatment modalities, physical findings, etc., and enters data into electronic database systems. 
    2. Abstracts Rapid Quality Reporting cancer cases (breast, colon & rectum) within three months of date of first contact to ensure reporting compliance. 
    3. Obtains and records information relating to the first course of treatment rendered to patients after discharge. 
    4. Codes required information according to the International Classification of Diseases for Oncology, ICD-0 manual, SEER summary stage, Site Specific Data Items, new Grade manual, Volume I and the STORE manual. 
    5. Stages cancer records according to the American Joint Commission of Cancer TNM Staging System, and the SEER Summary Staging System. 
    6. Obtains and records information relating to the first course of treatment to patients after discharge at outside physician(s) offices and/or outside facilities. 
    7. Knowledge of ACoS CoC standards for an approved Cancer program. Adheres to accrediting body guidelines for coding treatment methodology. 
    8. Demonstrates understanding of recognized Commission on Cancer quality measures of selected cancer sites. 
    9. Researches additional cancer related information not documented in medical record by contacting other physicians on the case and other facilities, institutions, etc. 
    10. Maintains the components of the cancer-related data collection system consistent with all regulatory and procedural policies. 
    11. Accesses protected health information (PHI) in accordance with internal and external institutional policies and procedures. 
    12. Attends cancer conferences and completes documentation attained at conference to include attendance by specialty, assigned clinical stage, clinical trial eligibility, genetic counseling and palliative care. 

    MARGINAL JOB FUNCTIONS: 

    1. Supports organizational goals and initiatives, ensuring the hospital's mission to improve the health of the communities we serve.

    EXPERIENCE / TRAINING:

    Minimum 3 years experience in medical records or other healthcare environment required or 2 years of Cancer Registry experience required. Completion of training in Cancer Registry profession. 

    LICENSES / CERTIFICATIONS: 

    Certified Tumor Registrar (CTR) certification through the National Cancer Registrars Association (NCRA).

  • Cancer Registry Assistant – Huntington Hospital, Pasadena, CA

     Under limited supervision, performs the activities of the Cancer Data Center to include case-finding, maintenance of complete and accurate data entry in accordance with the American College of Surgeons Commission on Cancer and the Cancer Data Center's operational standards. Obtains and prepares Huntington Ambulatory Surgery Center, and Radiation Oncology records for the case abstractor in a timely manner. Assists with cancer conferences. Reviews Radiation Oncology logs for new reportable cases by checking against cancer database. This position may require flexibility of hours.

     ESSENTIAL JOB FUNCTIONS: 

    1. Imports in the Cancer Alert System (CAS) pathology, cytology, and discharge messages on a daily basis. Reviews CT scans, nuclear medicine reports, and outside biopsies for reportability. Identifies reportable neoplasms, reportable CNS neoplasms, spinal cord tumors, brain tumors, and hematopoietic diseases. 
    1. Assigns appropriate accession & sequence number on reportable cases into a suspense file. Prepares monthly case list for abstracting by date of first contact. 
    1. Enters date and reason for admission into the database for existing cases that are considered follow-up. 
    1. Maintains the cancer database to ensure conformance to cancer registry operational standards and to meet the needs of the users of the registry data. Maintains high quality and accurate data entry. 
    1. Processes Radiation Oncology treatment summaries, and consultations. 
    1. Performs clerical support for the Cancer Data Center. 
    1. Interfaces with other departments, agencies, volunteers, and contract personnel. 
    1. Acquires Radiation Oncology patient records from Radiation Oncology Department for case abstracting. Promptly returns records back to Radiation Oncology after the case has been abstracted. Re-files patient records upon returning to Radiation Oncology Department.
    1. Processes treatment letters mailed to physician(s), other facilities, and/or clinics. Researches remote systems available for treatment rendered to our patients at outside facilities. 
    1. Responsible for the Rapid Quality Reporting System (RQRS). Consistently monitors RQRS case alerts and dashboard for cases missing treatment. Continually researches treatment to report to RQRS. 
    1. Attends cancer conferences and completes documentation to include, attendance by specialty, assigned clinical stage, clinical trial eligibility, genetic counseling and palliative treatment.

    EDUCATION:
    Completion of medical terminology, anatomy and physiology.

    EXPERIENCE / TRAINING:
    Minimum one year working in a cancer field required, or knowledge of cancer related diseases and/or experience working in the cancer related field.

     LICENSES / CERTIFICATIONS:
    CTR preferred.

    DEPARTMENT / UNIT-SPECIFIC SKILLS:
    Knowledge of anatomy, medical terminology, cancer diagnosis and treatment.
    Good oral and written communication skills.

    Abstracting: Identify analytic & nonanalytic cases within 6 months of diagnosis & collects data as outlined in the SEER/ACoS-CoC manuals and current State Cancer Registry Handbook. Abstracts a minimum of 5 abstracts per day (1abst/1.15hr). Minimum of 30 cases completed per week for remote abstractors.

    Accuracy: Must be committed to quality and the achievement of excellence by maintaining a 97% accuracy rate in abstracting,  and casefinding as required by the Cancer Committee Performance Improvement Plan.

    Follow-up: Identifies information necessary for a completed follow-up. Maintains follow-up according to ACoS Standards. Performs data entry of information obtained in the follow-up process.

    Attend at least one state, local, regional or national meeting annually to comply with ACOS and attend required educational training to stay current in data reporting requirements. Must be able familiar with National Treatment Guidelines for each specific type of malignancy.

    Maintains and protects all confidential information for HIPAA compliance, and in accordance with UTSW’s policies and procedures.

    Capable to work autonomously using accuracy in performing data abstraction and record reviews; uses attention to detail and organizational skills, able to work under pressure and prioritize tasks.

    Must be able to comprehend and multi-task between various hospital computer software and other data base programs; displays knowledge of Microsoft Word, Excel, Power Point.

    Performs other duties as assigned by the Oncology Quality & Outcomes manager and the Tumor Registry supervisor.
