Instructions for Prerequisite Verifications

Follow these 4 steps to complete your Prerequisite Verification Form.

STEP 1 Download and Fill out PDF Form

  Form A Prerequisite Verification FORM A: Verification of prerequisite and/or  corequisite(s) based  on successful completion of equivalent or external  coursework from regionally    accredited colleges, universities or high schools.  Examples  of coursework used for  Prerequisite Verification are: college and  university courses, high  school classes (foreign  language courses  ONLY); AP  and SAT test  scores.   Prerequisite Verification FORM B: Verification of prerequisite and/or corequisite(s) based on grounds other than coursework such as work experience, native language, license, etc. Provide valid reasoning for verification such as work experience, native language, license, etc… For more info on Verification Form B,click here.

PLEASE NOTE: Every semester has a deadline to submit Prerequisite Verification Form. It is the students’ responsibility to submit their form before the deadline, click here to view deadline and other important dates.

STEP 2 Gather Proof of Prerequisite Coursework

You must provide the following documents.

  • Form A: Must attach transcript(s) or verify transcript(s) have been received by SBCC
    • Must attach or have official transcript(s) on file that provide proof of completion with prerequisite and/or corequisite course(s)
  • Form B: Form has attached reasoning or supporting documents
    • If you do not attach a reason or supporting documents, your verification form will not be processed

STEP 3 Submit Completed Form with Required Documentation

    Tuesday & Wednesday: 8 a.m. to 6 p.m.
    Friday: 8 a.m. to 1 p.m.

    PLEASE NOTE: Transcripts are sent directly to: transcripts@sbcc.edu (Do not send transcripts to prerequisite's email, they will not be accepted).

PLEASE NOTE:  

  • Only complete verification forms will be processed
  • Verification forms are processed in the order received
  • Please do not send multiple forms or keep re-sending, this slows processing time

STEP 4 Check for E-mail Notification

  • ALL notifications and DECISIONS are emailed to students via their SBCC PIPELINE Email account
  • An approved petition does not guarantee space in any course
